Description

Description (Service) North Simcoe Muskoka (NSM) ABI Collaborative is comprised of four agencies, funded by Ontario Health, that deliver coordinated community Acquired Brain Injury services in North Simcoe Muskoka, four agencies are:
  • Brain Injury Services Muskoka Simcoe
  • Home and Community Care Support Services
  • York Simcoe Brain Injury Services
  • March of Dimes Canada
Services include:
  • In-Home Rehabilitation Supports and/or Nursing
  • Specialized clinical services to support community integration
  • Support for independence
  • Peer support and socialization opportunities
  • Adult Day Programs
The NSM ABI Navigator works locally to to promote and improve system access and capacity and improve outcomes and the client's overall experience. Areas of focus for the Navigator include Education, Resource Navigation, Complex Case Coordination, and the identification of system gaps and barriers

  • Must have a clinically documented brain injury
  • Must have a valid Ontario Health Card
  • Age Requirements: 16 year(s) and up
  • Persons 65 years and older are evaluated for services on an individual basis
