Description

Description (Service) Telehomecare supports patients with chronic obstructive pulmonary disease (COPD) or congestive heart failure (CHF) through health coaching and remote monitoring
  • patients are provided with easy-to-use home-based equipment, allowing them to monitor their own vital signs and answer simple questions about how they are feeling
