Description

Description (Service) Crisis Response Network Coordination provides individuals experiencing a situation or event that places them at risk with short-term support to resolve their immediate needs and mitigate more intensive interventions
  • Crisis Response Coordinator works immediately with the individual to develop a plan to address real-time concerns for safety and well-being and with other community service providers, in order to access needed resources
  • Support includes:
    • Case Management
    • Respite or Interim residential support
    • Access to Funding
    • Referrals for psychiatric support and future planning
    • Obtaining supportive housing and funding to participate in the community
    • Referral support to other community programs as requested
    • Participate in hospital discharge planning to safe environment for at-risk clients
    • Advocate and work collectively with community service providers, including police and Canadian Mental Health, Shelters, to reduce harm and risk for individuals who may be experiencing homelessness
This is not a 24-hour crisis service, persons requesting service will be contacted within two business days

Eligibility
  • Youth and Adults, 16 yrs and up with a dual diagnosis (developmental disability and mental health need and/or challenging behaviour)
  • Youth (16 years of age and older), with a dual diagnosis, who are involved with the justice system may be eligible to receive up to four months of case management support to assist them with legal services through Crisis Response Network Coordination
