  • When called back arrangements for Intake will be made
  • Intake will take place in clients home, in addition to a home assessment
  • During Intake provide notice of assessment (line 15000)

    Fees Service- call for fees. Fee assistance available for seniors or adults with physical disabilities with limited means. Individuals earning less than $33,000 (line 15000 notice of assessment) are eligible for up to 50% reduction in service fees.
