Description

Description (Service) Provides in home and in clinic nursing care and in home personal support care throughout the North Simcoe Muskoka Region
  • Ambulatory Care Clinics are located in Barrie, Orillia and Midland
  • PSW level or Nursing level geriatric trained staff are certified to provide care to clients with Acquired Brain Injury and to clients with Dementia
  • Nursing Care Services include:
    • palliative care
    • pediatric care
    • wound care
    • mental health care
    • post-operative care
    • medication management
    • health teaching including wound and stoma therapy advice and Nurse Continence Advisor services
    • physical, mental health and mobility assessments
    • medication and nutrition review
  • Personal Support Care services include:
    • light housekeeping
    • meal preparation
    • personal care
    • friendly visiting
    • companionship and accompaniment to appointments
    • respite care
  • FALLS PREVENTION:
    • Fall prevention assessments in the client's home
    • Member of Simcoe Muskoka Integrated Fall Strategy (SMIFS)

  • No restrictions for most services
  • Respite care: families of Alzheimer patients, developmentally handicapped, severely disabled as well as frail elderly

Languages English; French - Programs, Have nurses, PSWs etc who speak French; one night staff member is bilingual, select Administrative Staff are bilingual
Fees Service - accepts Veterans Affairs of Canada K number; also provide care funded by WSIB and extended health care
