Description

Description (Service) Partnership between the March of Dimes Canada and Mackenzie Health provides support to individuals who live with the effects of an acquired brain injury
  • Team includes Behaviour Consultants, Case Managers and Rehabilitation Workers
  • Services include:
    • case coordination and comprehensive functional assessment to evaluate the effects of the acquired brain injury on the individual and those who support them
    • individualized service/treatment plan, developed with recommendations and community support to promote the acquisitions of skills and strategies
    • plan which will enable persons with an acquired brain injury to live with maximal fulfillment in a state of dignity within the community
