Description (Service) Home health care services including:
  • Nursing Care
  • Caregiver Relief
  • Pediatric Care
  • Elder Care
  • Palliative Care
  • ET Nurse
  • Continence Advisor
  • Post-Operative Care
  • Chronic Disease Management Education
  • Personal Support Services such as Companionship, Dementia Care, Physiotherapy and Exercise Programs
  • Falls risk assessments, work with clients to further reduce the risk of a fall by providing:
    • Nurses and PSW workers to manage and educate clients on individual medical risk factors such as Diabetes, advanced foot care and bladder and bowel incontinence
    • Visiting nursing to monitor medications use
    • Product advice and help with product use, including assessments for assistive devices
