Description

Description (Service) Seniors to Home program for older adults facing complex challenges who could safely live in the community with access to subsidized housing and enhanced assisted living services
  • program designed for older adults that meet Alternative Level of Care criteria at hospitals within the North Simcoe Muskoka LHIN
  • accommodations are shared and there is a charge for accommodations based on income
  • Domiciliary Care Funding may be available, call for more information
  • rooms are fully furnished, including bedding, TV sets, A/C units
  • cable and phone is not included in rent
Community Services include:
  • enhanced support (personal care, medication support, life enhancement and case management provided to program participants
  • housekeeping
  • meals
  • escorts to appointments
  • social activities and leisure programs
  • crisis intervention support
  • behaviour support services
  • 24 hour emergency response
  • referrals to specialized services

Eligibility Admission Criteria:
  • Older adults and seniors (55+) , those under 55 who meet the criteria below will be considered on a case-by-case basis
    • Designated ALC (Alternative Level of Care)
    • A need for permanent housing with specialized assisted living supports
    • Facing complex challenges which include mental health and/or addiction challenges, dementia, ongoing health issues, social determinants of health needs, responsive behaviours an/or other neurological disorders
Application
  • Referrals accepted from hospitals within the NSM Home and Comnmunity Care
  • email completed application and consent form can be sent to rirfan@loftcs.org or faxed to 705-436-8514
  • if client deemed eligible, an in-person interview will be scheduled to assess suitability for the program
  • tour of premises prior to moving may be offered
