Description (Service)

ILS assists adults with permanent physical disabilities and seniors with activities of daily living such as bathing, transferring, meal preparation, and household routines:

ASSISTED LIVING SERVICES

  • Services are designed for client who may require more frequent access to service on a 24-hour basis that cannot be met solely on a scheduled visitation model.
  • Clients must live within a designated geographic service area

Eligibility To be eligible for Assisted Living Services you must meet the following requirements:
  • Be an individual with a physical disability or meet characteristics of a High-Risk Senior (HRS) as identified in the profiles of Assisted Living Services Policy
  • Be insured under the Health Insurance Act of Ontario
  • May require unscheduled services for personal care that cannot be met by a prescheduled visit.
  • Require assistance with homemaking and activities of daily living, such as bathing, dressing, toileting and transferring that cannot be me solely on a scheduled visitation basis.
  • Live within the defined geographic area within Simcoe County.
  • Be able to direct their own care or have a live-in caregiver/decision-maker to direct care
  • Be able to remain safely in their home between visits.
  • Are medically stable and have their medical needs met by an existing community health networks.
  • The home has the physical features necessary to enable services to be provided safely
