Description

Description (Service)

A Community Treatment Order (CTO) is a physician's order for community based treatment for individuals living with a serious mental illness who have had repeated hospital admissions and require ongoing support when they leave the hospital

  • a CTO ensures that individuals receive all the supports they require to live in the community including a physician, a case manager, housing, support with medication and assistance with finances and meaningful activities
  • CTO Coordination Service provides support to every CTO that is issued in LHIN 12 Region (Simcoe Muskoka)
  • support includes: assessing clients' suitability for a CTO, assisting physicians to complete the proper forms and advocating for services for those individuals meeting the criteria for a CTO
  • service also provides education and information about Community Treatment Orders, Mental Health Act and related legislation, to any interested individual or group as requested
