Description (Service)

North Simcoe Muskoka Specialized Geriatric Services (SGS) are provided through local specialized teams across the region that specifically focus services on the care of older adults living with complexity and frailty and their care partners

  • Central Intake is the hub making access to services easier by creating a single intake service, includes:
    • Geriatric Medicine
    • GeriMedRisk
    • Geriatric Mental Health
    • NSM Seniors CARE Exercise Program
    • Complex Case Resolution
    • eConsult

  • single consolidated SGS Referral and Medical Clearance Form, must be faxed or securely sent to SGS Central Intake to protect personal health information
  • once a referral is generated, additional SGS resources may be accessed based on individual patient needs
