Description (Service)

Volunteer based organization, providing client centered support for those living with a life limiting illness or grieving the loss of a loved one

  • Services for clients, caregiver(s), and family members address physical, psychological, social, spiritual, and practical needs
  • Services include:
    • Residential Hospice
    • In-home Visiting
    • Bereavement Support
    • Care for Caregivers
    • Pregnancy and Infant Loss Support
    • Day Programs
    • Children & Youth Support
    Annual Hike & Bike for Hospice fundraiser will be held on Sun May 5, 2024. See website for details.

Residential Hospice and Visiting Hours: Mon-Sun 24 hours

Application Residential Hospice
Assessment is required by Home and Community Care (HCC) prior to admission:
  • Referral request must go to Home and Community Care (HCC), phone: 705-721-8010
  • The request can come from a physician, client or substitute decision maker for the client
  • HCC will complete an assessment and referral prior to admission to Hospice Simcoe, admission is based on a triage assessment tool that identifies the person most needing an admission
