Description

Description (Service) Focuses on moving people who are chronically homeless as rapidly as possible from the street or emergency shelters into permanent housing with supports that vary according to participant needs.

Eligibility Available to people who:
  • Are currently experiencing homelessness
  • Meets the definition of chronic homelessness
  • Have been assessed (VISPDAT) having a high level of acuity
  • Lives in Simcoe County
  • Has been added to the By-Name List
Application Call 2-1-1, shelter or outreach to apply.

In order to apply for the program you must;
  • Complete a Common Intake and Consent form
  • Complete a common assessment (VI SPDAT)
  • Have your request added to the By-Name List
