Description (Service)

Provides two transportation programs for residents unable to use public transit due to a physical, cognitive, visual or sensory disability

  • Transportation Linking Communities (TLC)
  • Midland Accessible Transportation (AT)

Services offered include transportation to:

  • vaccine appointments
  • regular medical appointments
  • specialized health services appointments such as dialysis or chemotherapy-local and out of town
  • shopping
  • social events

Runs in partnership of transportation providers including:

  • Helping Hands Orillia
  • Barrie Are Native Advisory Circle
  • Canadian Red Cross
  • Community & Home Assistance to Seniors (CHATS)
  • Community Reach

Eligibility Vary, eligibility is established upon completion of medical assessment form and submission of transportation application form to Community Reach.
Application The Medical Assessment Form completed by a Health Care Professional along with the Transportation Application Form must be submitted to Community Reach to establish eligibility.
